The table below describes how to complete the application Form 128 Application for renunciation of Australian citizenship.

Your details Part A requires you to provide your personal details. If you change address during the application process you must tell the department your new address.

B
Citizenship details Part B requires you to provide information about your current citizenship.
C
Your child If you are the responsible parent applying on behalf of your child aged under 16 years, Part C requires you to provide further details of your child.
D
Proof of identity Part D requires you to list the documents you are providing to prove your identity.
E
Identity declaration Part E is not to be completed by the applicant. Ensure the identity declaration is completed by someone from the designated list of occupations.
F
Supporting documents Part F requires you to list the supporting documents you are providing with your application.
G
Nomination of representative Part G allows you to nominate another person who can make enquiries or receive information about your application for you. Make sure you sign the authorisation at Part G if you want this to happen.

Payment details

Check the current fee for this application.

In Australia – Debit card or credit card are the preferred methods of payment. Debit cards cannot be used for applications lodged by mail. If paying by bank cheque or money order please make payable to the Department of Immigration and Citizenship.

Outside Australia – Credit card payments or Australian dollars (AUD) bank cheques are accepted. If you do not have access to a credit card or AUD bank cheque, contact your nearest Immigration Office outside Australia to find out about alternative forms of payment.

Declaration Carefully read and make sure you understand the terms of this declaration before signing it. If the applicant is under 16 years of age, the responsible parent must sign the declaration on behalf of the child.
